Health Forms

Creative Summer adheres to all health guidelines and requirements set forth by the Maryland Department of Health and the Montgomery County Health Department. In accordance with health department policies, Creative Summer must have the proper health forms on file for each camper. Please review the Health Form descriptions below to determine which forms, if any, are necessary for your camper.

All Supplemental health forms must be submitted to the Creative Summer Office by Friday, April 30, 2021.

Health forms posted here were current January 21, 2021, but may be subject to change if the Maryland Department of Health updates any requirements.

REQUIRED COVID TESTING

All Creative Summer campers, staff, and on-site employees are required to submit documentation of a negative COVID-19 test taken no earlier than the Wednesday before the start of each session of camp. Test results can be uploaded to your camper's online account or emailed to the Creative Summer Office by 6:00 p.m. the Sunday before the start of each session.

Information about on-site COVID testing will be emailed to registered families on or before June 1, 2021.
