Health Forms

Creative Summer adheres to all health guidelines and requirements set forth by the Maryland Department of Health and the Montgomery County Health Department. In accordance with health department policies, Creative Summer must have the proper health forms on file for each camper.

Please review the Health Form descriptions below to determine which forms, if any, are necessary for your camper. If you choose to submit any optional forms, please make sure they have all required signatures, including physician signature for OTC forms.

IMPORTANT: We can accept health forms ONLY in the format provided on this page, as these are the forms required by the Maryland Department of Health. We cannot accept health forms from any school system or other organization; they are not considered valid by MDH.

All Supplemental health forms must be submitted to the Creative Summer Office by Friday, April 29, 2022.
